As a Telephone Case Manager RN with UnitedHealth Group, you'll support a diverse member population with education, advocacy and connections to the resources they need to feel better and get well.
Key Responsibilities
- Able to type and can navigate a Windows based environment
- Identify gaps or barriers in treatment plans
- Active Multistate Compact Nursing License with primary residency in the state of residence, allowing the employee to practice in Louisiana and their home state
- Experience in a telephonic role
- Certified Case Manager (CCM)
- A background in managed care
- Aides or Meals on Wheels
- Make referrals to outside sources
- Case management track record
- Background or exposure to discharge planning
- Make outbound calls and receive inbound calls to assess members' current health status
- Provide patient education to assist with self-management
